Hello everyone,
I am a first year nursing student currently working on what seems like an endless stream of Nursing Care Plans. Trouble is, it takes me a really long time because I just don't get it. My professor wants us to write a plan on one diagnosis, with one goal, and two to three outcomes. I am having trouble coming up with more than one outcome relating to the same goal/diagnosis. I have multiple diagnoses (i.e. Activity intolerance, anxiety, impaired skin integrity, etc.) which all have their own etiologies. I guess what I don't understand is, do the outcomes focus on the etiologies or the diagnosis itself? If it is just the diagnosis, then I am simply having trouble relating more than one outcome to that diagnosis.
Any suggestions are welcome, and MUCH appreciated!

outcomes and goals are the predicted results of our independent nursing actions. look at the etiology that is the cause of your nursing diagnosis and what your nursing interventions are doing to (1) improve, (2) stabilize, or (3) support its deterioration as well as what interventions you have developed for the symptoms that might be evidence of it. you should be able to express those predicted results in terms of reversed or improved symptoms.
