Nursing Care Plan

I need help with my nursing care plan. I have to have two nursing dx over my pt. He was admitted for dehydration and weakness. I dont really know where to start. I know one should be deficient fluid volume.

Do you get a Nursing Dx book in your program? Impaired Oral mucous membrane r/t fluid deficit could be one. Fatigue could be another but the dehydration is probably the cause for the weakness.

lucky you! we have to come up with 10 diagnosis or risk for Dxs. I wish i could help but I need help with mine too lol

The easiest thing to do, is to look in your nursing diagnosis handbook. If you do not have one, buy one!! I have Ackley, and it makes care plans so easy. What you do is look up the patient's medical Dx in the front. They are listed alphabetically. Under each medical diagnosis there is a list of nursing diagnoses. You can look through & decide the most appropriate for you patient. Then you flip to the diagnosis you have chosen in the back portion of the book. That is were they have listed sample goals, criteria, interventions, teaching etc.
