Is it really necessary to put a quotation mark in making a subjective data?

Are we allowed to rephrase the patients verbal data?

IF yes, can we put the rephrased sentence under the subjective data?

Another Question (Sorry this is My first time in making A nursing care plan)

Is Discharge goals the same with Objectives?

Or

Is Desired Outcomes the same with objectives?

I got confused because the NCP book contains both discharge goals and desired outcomes but no objectives. Our nursing care format Only requires Objectives

is it really necessary to put a quotation mark in making a subjective data? only if the patient actually makes the statement and you want to use it because it makes the point of the evidence (symptom) you are trying to show.

are we allowed to rephrase the patients verbal data? yes

if yes, can we put the rephrased sentence under the subjective data? yes, you must.

are discharge goals the same with objectives? don't know. check with your instructors.

or are desired outcomes the same with objectives? don't know. check with your instructors.

i got confused because the ncp book contains both discharge goals and desired outcomes but no objectives. our nursing care format only requires objectives goals and outcomes are usually felt to be the same things--anticipated results. check with your instructors to be sure.
