My son kept me up all night and I wasnt able to concentrate, now im running out of time and this Hmwk is due Tomorrow so please If anyone can help me out...Thanks!!
An 85 year old female patient was admitted to the hospital as a result of a fall . Early tests show that she may have had a mild CVA and now has right sided paresis with expressive aphasia. Vital signs are: BP 130/72; TPR: 101 degree's F; 110; 26. Additionally, she wears hearing aid's, but left them at home. The nurses' notes state that she is confused and that her speech is slurred and difficult to understand. She is on a pureed diet, but has only been eating about 20% of her meals indicating that she is just not hungry. Her intake and output for the past 24 hours is 800 cc of oral intake and 700 cc's of concentrated urine output. According to family members, she has not had a bowel movement for 5 days. There is a reddened area on her coccyx area. Also, she is running a low grade fever of 100 degrees orally and has a WBC of 11,500.
1. What are possible reasons for elevated temperature, pulse and respiration?

Her elevated temp, pulse and resp along with elevated WBC's seem to indicate infection-viral or bacterial...also the low I/O and lack of BM indicates possible dehydration
Hearing loss and confusion relationship-could be as simple as she is confused b/c she can't hear, or could be dementia or r/t CVA...
Pureed diet is what is says and is ordered like for dysphagia from the paresis
Normal I/O is generally 1200-1500mL
Nursing care for Stage 1 pressure ulcer, positioning q 2, barrier cream, frequent observation...
For your cluster/Nsg Dx I would likely use the most obvious information given so that you can make a diagnosis with goals that are achievable.

no. practice better time manage and attempt to do your own homework. where do you live? because, i do not ever want to be cared for by a nurse who had their homework done for them on the internet all through nursing school, thanks.
