nursing assessment forms

Does anyone know of a website or has a good assessment form that i can use when I make my rounds. We have computer charting and its hard to remember all of my assessment when I have to leave the room and chart. I was hoping that there was a good paper form I could use when I was in the room. Thanks.

I would be interested in these to use with new nurses I precept. I have one but a different type may work better..............

I would ask the supervisor/director/charge nurse if I could make copies of the forms they use when the computers are down. Or you could just take the time some day and write down the areas you need to assess and chart on and type that up and take it with you room to room.

I have a nurse friend who draws something like an exaggerated stick figure, with legs and arms sorta splayed out-she puts the lung sounds for each field on there, and pulses, etc. It works well :)
