IF you go to the online bookstores and put in searches for Medical Spanish, you'll get a veritable snowstorm of returns. I formerly used a spiral bound book called Survival Spanish for the Medical Professional by T. P. Skaarup but it looks to be out of print and available from some used sellers at Amazon.
Netty,
I really liked TP Skaarup's book. He was a internal med resident (probably in the 80's) and created his book out of his real life desire to communicate with people. He has cultural notes (a few) in the book, too. His book was designed to be used by non-speakers and has English and Spanish side by side and would probably be a great augment to you BUT I bet so would any of the gazillions of products that are currently on the market. Skaarup's book was spiral bound and sized to fit in the large pocket of a lab coat (about the same size as a Washington Manual or a Harriet Lane Manual).
Happy hunting; there's tons of products out there to meet this gap, so don't necessarily buy the first thing you meet.

Can Anyone recommend a book that can help me translate questions regarding assessment/ patient history in Spanish?
I work at a MMTP clinic where a vast majority of the patients speak spanish. I am Hispanic, but my knowledge of medical terminology in spanish is LACKING!!!!!:imbar. preferrably one that states the questions in both English and Spanish.
I would be ETERNALLY GRATEFUL!!!!!:kiss
