That's awful. But, it doesn't matter how experienced a nurse is, mistakes get made. Never be so comfortable in the "experienced nurse" role to think that you won't make a mistake. One of our best nurses, who was having one of those days that you never forget, hooked up a feed to the balloon port of the g-tube..this is before we had the feed systems and rigged up things. She felt absolutely awful.
As for the feeding tubes, I'm sure many countries aren't required to use the same systems we are, that is why we have sentinel events here and they use those to make change.
