Hello to any risk managers or anyone who else conducts RCAs
Question...we have a patient safety coordinator who is otherwise very good at her job. However, whenever a new RCA is needed, I get an email saying "hey need you to cough up 3 people from your department every Friday for the next 5 weeks for a two hour meeting to get this RCA done". Maybe this is just the way it's done, but I've talked to at least one other nurse that does these, and she says that is probably overkill and much of the work can be done in between meetings. As a manager, it's hard for me to comply with this, since my staff work different schedules/shifts/take leave, etc etc. Anyone else have a different model on how to do this more time-efficiently?

In my experience, it's the leadership that does the meat of the work, and they will invite floor nurses as subject-matter experts for *a* session for an hour or two.
