Hi I'm a nurse from the Philippines doing graduate studies. Nurse Practitioner Programs are not very popular here yet but we are taught about it in grad school. I need help in finding out what are the Parts of an NP's prescription pad and if anyone can provide me a picture I can show in class. Thank you very much.

We use the same prescription pad as the Dr.'s do. Mine has our physicians name along with all of the NPs that work at our office. A place for the patients name, date, date of birth and then a big blank area for the name of the drug and the instructions for dose, etc. and a signature line at the bottom.

I'm in IL and our practice uses e-prescribing: I type in the script into the computer and its sent electronically to the pharmacy.
