New school nurse in Ca, desperatley in need of forms!!

Hi. I am a new school nurse in California. My position is new at this district so I am starting from scratch and I am hoping some of you experienced school nurses out there would be willing to share forms/templates that you have found useful. I am specifically looking for Health update assessment forms for Triennial IEPS, health Hx forms for initial IEP's, and any ISHP/504 templates you have for seizures, ulcerative colitis, hypoglycemia, diabetes or any other ones for uncommon conditions that you have come across. Also, if you can think of any forms that I may not be aware that I will need yet, those would be gladly accepted!! Thank you, my email is [email protected]

Welcome! Congrats on your new position. This book was a great investment and what I used for all my health plans. There's also a CD with the plans computerized. Easy to edit and tailor to each individual student.

Look on CHOA.org. Children's Healthcare of Atlanta has a great School Nurse manual available for download on their site. Just look under nursing resources - there should be a section for school health. The manual has lots of forms in it.
