Ok mainly i wanted to know.. It sounds stupid but what do you call and tell the MD when there's an admission ? Just that they have arrived ? How do you do med reconciliation ? Does the hospital provide a list of meds and then u read them off to see which meds he wants to continue?

WELCOME to allnurses! Wow sounds exciting! All places are different, they should put you through an individualized orientation and/or assign you to a nurse who will walk you through these different tasks. If not, talk to your charge nurse and let her know what you need. Again, it is different in each facility. Hope it all goes well and keep posting!

You should have a facility medication reconciliation that will need to be filled out.

We also have preset admission orders that the MD can check off. Things such as admission blood work, swabs (although everyone gets those), TB skin test etc. Our doctors usually wait to fill those out during their weekly rounds. When I call them for admission orders, I usually say that we have a new resident, give them a brief health history (ie hypertension, diabetes, CHF etc) and ask if I can go over the medication list I read it out and get a verbal order of what medications to order as well as take a verbal order of any changes to the medications. Most home medications are continued and then during the weekly rounds some are changed once the MD assesses the resident. I also get the medication directives order to give PRN meds (laxatives, Tylenol etc).

Check with what other nurses are doing at your facility as there may be a policy to follow.

We have an admission check list we use. I look at the home medications that are sometimes listed, not always accurate though. I also look at the meds from the hospital. We have standard admission orders that we have received the OK from the MD to put into play for the majority of our residents. Most of the time our MD just wants to know if the resident arrived and will be in that day or the next to see them. If there are any major issues I'll bring those up at the time I speak with the MD.
