Needs some help putting togehter a LTC resident satisfaction survey.

Hey! I'm doing a QI project for leadership, and decided that I would like to do a conduct a survey at a LTC facility. I'd really like to make this good. Anyone have any good ideas? Or sources to get some?

Big things with my residents, because they have control over so little:

They want food choices.

They want help to the toilet/bedpan faster.

Ok, thats definitely something I'll address. Such as "are you satisfied with the quality and variety of the food provided at this facility" and "are your toileting needs met in a timely and adequate manner" Other things I think it would be good to ask would be cleanliness of the facility, knowledge and friendliness of staff.
