I'm a nurse working in clinical informatics. For my MSN practicum in Population Health I'm working with our IS Population Health and Community Systems Manager.
She has tasked me with compiling a comprehensive list of patient centered goals for outpatient use by case managers. I've searched the internet high and low and have come up with nothing. I am really hoping I don't have to recreate the wheel here... there has to be something in existence already similar to the NANDA list of nursing diagnosis. Our EHR only has 60 patient centered goals for outpatient use, and a plethora of goals on the inpatient side. These are acute care focused though, and aren't necessarily patient-centered or appropriate for longterm use.
The end result would be for suggested goals to trigger an alert to case managers in our EHR based on diagonsis on the problem list.

I used to work in IT a long time ago and what you are asking will require you to do more than look stuff up on the internet. I understand that sometimes one can get information off of the internet to tackle a project, but some information is highly valuable! With that said, there are organizations and software vendors to provide what you are seeking. If your company is trying to do this for less than purchasing software or hiring a consultant (i.e. recruiting you to do the work), than you will need to work and work hard!
This means stepping away from your computer and your desk to learn the assessment role of an outpatient case manager. Your best bet is to interview your outpatient case managers and read lots and lots of journals to get a better idea of patient centered goals for outpatient settings. Otherwise, propose that your company hire a consultant or purchase software (or additional add-ons that are currently available with your current EHR) to make this information accessible to your organization. I suggest that you talk to your IT people. Let them know your problem and maybe one of them can help you to talk to a contracted vendor and discuss the upgrades for current EHR without re-inventing the wheel.
