  3. I'm a nurse working in clinical informatics. For my MSN practicum in Population Health I'm working with our IS Population Health and Community Systems Manager. She has tasked me with compiling a comprehensive list of patient centered goals for outpatient use by case managers. I've searched the internet high and low and have come up with nothing. I am really hoping I don't have to recreate the wheel here... there has to be something in existence already similar to the NANDA list of nursing diagnosis. Our EHR only has 60 patient centered goals for outpatient use, and a plethora of goals on the inpatient side. These are acute care focused though, and aren't necessarily patient-centered or appropriate for longterm use. The end result would be for suggested goals to trigger an alert to case managers in our EHR based on diagonsis on the problem list.
