I have to come up with a NANDA dx for a newborn baby. The baby was healthy. Labor was induced at 37 weeks because mom had preclampsia. Baby has no trouble breathing, APGAR 8 then 9. No delivery problems. As far as I know baby is good. What dx do you give for a healthy baby? I have to have six interventions but I can't even think of what dx there is for a healthy baby? Please help???

Readiness for Enhanced Knowledge of Newborn Care r/t mother's expressed interest in the newborn's needs (if she expressed any)
Risk for Infection r/t newborn's unhealed umbilical cord
Thanks for your response, but I have to have a dx that is related to baby and it can't be risk of because I need interventions and evaluations.

We always use this for our health newborns
Diagnosis:
Risk for impaired thermo-regulation r/t the newborns inability to self regulate temp
goal: to maintain temp above 98
nursing orders:
1) assess temp q 4 for 24 hours and q 8 there after
2) if temp below 98 put under warmer until temp above 98 x 2
3) wrap infant in 2 warm blankets
4) change diapers frequently
5) warm all items before contact
6) reduce drafts in infant areas

Also you could use risk for dehydration r/t learning to breast/bottle feed
Risk for impaired bonding r/t whatever (traumatic birth, mothers illness, maternal/child separation)
Impaired Skin Integrity related to newborn's attached umbilical cord
Altered Family Processes r/t the addition of a new baby to the family
