Need help

Hi so I have to come up with a diagnoses and some interventions with time frames on a pt I had with dementia. She is nonverbal so The diagnoses I did on her was 

impaired verbal communication rt disease process amb difficulty expressing thoughts verbally

Al I need is a time frame so what should I put if my intervention is to assess for presence of expressive dysphasia and receptive dysphasia?  Should I say during shift, upon interaction?

    This is not an appropriate intervention, it is the assessment that you have already done. You have chosen a nursing diagnosis. What kind of diagnosis-related goals are appropriate for this situation?
