Need help with nursing diagnosis

I am a current LPN student. We as a class are just getting our feet wet with care maps and nursing diagnosis'. That being said we had to pick a nursing diagnosis ( I chose CHF: excess fluid volume r/t impaired excretion of sodium and water) I need help on how to form a care plan. I am so lost :) Any help would be much appreciated

You should have a nursing diagnosis book that came with your books. If not please invest in one, they really help. Look in the index for whatever you are doing your care plan on and it will give several page numbers. On those pages you will see interventions, plans, assessment tools, etc. Good Luck!!
