Hey, i'm doing a care plan on a Schizophrenic patient right now. My NANDAs are as followed:
Risk for injury
Disturbed personal identity
Disturbed thought process
Dressing/grooming self-care deficit
Situational Low Self-Esteem
Need to put those in priority order from #1-5, any help would be greatly appreciated!!!!!

I was taught prioritize according to Maslow and once in "Maslow" order, priorize those into actuals and then risk for.

You need to prioritize with your ABC's (airway, breathing, circulation) first and foremost, and then Maslow's.
Maslow's is by ABC and then all the other ones.
