I have a skills package and I need to make a care plan for a 48 yr old who had stage 3 breast cancer. She just had a radical mastectomy. She has 16 staples with 2 sutures holding the drain in place. She also has a portacath in upper left chest. Shes also a half pack a day smoker. Thank-you!!

The skills package is for skills testing. We get case scenarios and we have to have pathos, care plans and plan of care. I'm stuck on the mastectomy.. I know two diagnosis that will good for her: Acute pain r/t surgical incision AEB mastectomy and Risk for infection r/t surgical incision AEB mastectomy. I need to have 4, I'm stuck on having two more.

Ineffective Peripheral Tissue Perfusion, Risk for Fluid Volume Deficit.. would those be good relating to her mastectomy?

Have you given any thought to a nursing diagnosis that would address the psychosocial aspects of her care?

disturbed body image? would that be a good one?

Yes, that's a good one.

Also think about the whole picture, not just the surgical wounds - cancer, smoking (how does it affect healing, etc?), family, sexuality, teaching points...
