:heartbeat I'm a few weeks away from finishing my second semester of a BSN program, and I'm having problems with head to toe assessment. My school doesn't have a specific form we have to use for the assessment. My clinical instructor is not pleased with the way me and my class mates have been doing assessments. We do have a textbook for assessment, and it's helpful most of the time. Let's just say the instructor gets agitated when we ask her too many questions. If anyone has some advice for me I would appreciate it.

allnurses has a thread with a number of weblinks to assessment information. some of them include links to videos.
i have to tell you, however, that assessment is something that you are always learning something new at. you also, most likely, will have to create your own head-to-toe assessment form. there is a 6 or 7 page assessment form in the center of taber's cyclopedic medical dictionary in the listings for "nursing". it's a start. the student clinical report sheet for one patient attached at the end of all my posts has a review of systems assessment on it. click on it to open the file so you can download and copy it.
