Need help with exam revision; 24 year old Muslim with viral pneumonia

Need help with a written exam that I have coming up in the next few weeks.

The scenario is of a 24 year old Muslim with Viral Pneumonia.

I need to select three activities of Daily Living using Roper Logan and Tierney model.

I have selected breathing, mobility and communication.

For each of these three topics I need to cover Assessment, Planning, Interventions and Evaluation. Would it be possible for someone to do me an example of say Mobility as I am struggling :/

i don't think "breathing" counts as an adl, although if he stops, that will be the end of all his adls.

i'm not sure how you are struggling here. is it terminology? seem to me if i had to assess someone's mobility, i'd look at his gait, endurance, and safety factors. if i found difficulty with any of those, i'd plan some care or compensatory measures and future targeted assessments (meaning, what specifically would i look at, and how often?). evaluations are, well, evaluations: how well did my interventions work, or do i need to tweak them?

what am i missing here? can you explain what you've got so far and what confuses you?

Well for mobility I've got this so far

Assessment

Mobility is restricted due to patient being dyspnoea

Plan

To improve patients mobility

Interventions

May be at risk of skin breakdown, pressure sore development; complete a daily waterlow and check relevant areas for pressure sores ankles, buttocks, knees and elbows

Prescribe relevant pressure sore equipment; mattress and cushion

Make sure the patient is getting the right nutrients which will help heal wounds, checks menu's and keep a food chart

The nurse should aim to turn patient every 2 hours to prevent skin breakdown

Patient is at risk of DVT observe calf's for swelling and tenderness.

Doctor may need to prescribe clexane and query Ted stockings

Refer patient to physhiotherapist for early mobilisation and leg exercises to prevent DVT's

i don't see anything there that addresses his dyspnea (us spelling :D). if his decreased mobility is due to being dyspneic, is there something that you should be looking at in that regard (assessing, modifying...)?

what adls might be difficult for him to do for himself because he gets short of breath? what could nursing do to address those?

if he's 24, is it fair to assume that he is probably not so disabled that he can move himself in bed a bit? in that case, the nurse may be able to explain to him about ....

is his language and cognition such that nursing can be teaching him (about.....?)? (let's assume "yes.")

how do you assess calves (plural; not possessive) for dvt? swelling and tenderness, yes, but have you learned about homan's sign? if not, look it up and incorporate in your assessment

normally instructors want students to talk about nursing activities. can a nurse instruct a patient on early mob and leg exercises? of course. why wait for a pt to come?

do you have the nanda-international book of nursing diagnoses? each one has a list of defining characteristics (the "how i know the patient has...") and just reading them will give you great ideas on how to nurse him (and anyone else).

Hi Kate. I am in your year and also about to take the exam. The problem i have been having is wondering if i am going along the right lines of what to revise for the exam with us only finding out the questions with regards to him when we take the exam and also when i am going to answer them if i am putting the right answers to the right questions as they all seem to overlap eg care delivery and care management
