Need some help and advice

Hi, I am looking for some info on hiring a CNA for private duty. My grandfather has terminal cancer and we have a person in mind that we would like to have come to his home and help out with bathing, meals, laundry, etc. Can we legally write up a contract for her to sign stating the hours, rate of pay, duties etc and have both the CNA, my grandfather, and my mother (his poa) sign? How do we handle taxes for her pay? I don't really have want to hire through an agency and have a stranger coming to his house. I have had some bad experiences agency sitters that families have hired at the facility that I work at. We want someone who we know and can trust. By the way, we live in Illinois. If anyone has any info or can refer me to the proper people to contact it would be greatly appreciated.

When I worked private duty I did not have a contract but my employer had her CPA do my taxes and paycheck. You can do the payroll taxes for your employee or you can pay them X amount and make it very clear to them that they are responsible for their own taxes, thus putting the onus on them to get help if they need it. I am fairly certain that if you choose to do it this way, you should issue them a 1099 at the end of the year, rather than a W2. You can get booklets from the IRS that talk about all of this.
