My job said I need to do my "Obra charting" but no one at work seems to know what that means. For example, it says 3-11 shift do charting for pain on room 601A. On 11-7 shift it says do charting for bed rail on room 603B. What does that mean? Do I chart in the nurses note that patient has pain or not? What about bedrail? Do I chart that the bedrail is up or down? What else? If anyone could show me "actual things I can say in my obra charting" in the nurses notes that would sure help out. Thanks for any assistance.
