Need assistance with Health Promotion

Hello Everyone!

I am new to this forum thing but I was wondering if anyone could possibly help me with my health promotion paper. I have a client who is 93 yrs old. He has psoriasis, but only on his head. He has poor nutrition, because he is a very picky eater. I need to come up with a nursing diagnosis, related to skin integrity and a health promotion plan in order my client to participate in healthy behaviors. Maybe incorporate the Brayden Pressure Sore Risk Scale? I have emailed my teacher because I am a little confused of what is expected of me but I thought I would get some other persons opinions in the mean time.

the first thing i would do is read about psoriasis and how it is treated. you can't develop any teaching plan if you don't know anything about this disease or how it is treated. then, look at websites like medline plus (you will find the weblink here: https://allnurses.com/nursing-student-assistance/medical-disease-information-258109.html - medical disease information/treatment/procedures/test reference websites) which have consumer health information on them to see what information they may have for the public about psoriasis and see what you can construct as a teaching plan for your client. as always, when doing this kind of assignment you still follow the nursing process. it is our problem solving tool. first assess. assessment consists of:

  • a health history (review of systems)
  • performing a physical exam
  • assessing their adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition
  • reviewing the signs, symptoms and side effects of the medications they are taking

then, you move on to diagnosing (impaired skin integrity). third step is developing the goals and plan of action (interventions). specifically, teaching plans include:

  1. overview: a synopsis about what is going to be taught in the course
  2. goal(s): the aim(s) or outcome(s) that you want your learner to achieve as a result of the lesson you plan
  3. objectives: the more specific information that the learner will come away from the course knowing that will achieve the goal(s) you have determined.
  4. content: a play-by-play of the specific content that is going to be taught and in the sequence it will happen. your content should address and cover all the objectives. this part of the written lesson plan is presented in an outline format.
  5. procedures and materials: how all the above will be achieved, i.e. lecture, demonstration, discussion, etc. materials that can be used and resources that can be needed for the lesson to be successful and essential to teaching your lesson plan are listed and may include demonstrations, audio-visuals, handouts, experiments, stories, game playing and any number of other creative items.
  6. evaluation: determining if you met the goals of the teaching plan. this can be done through a return demonstration, short post test, short question and return answer session with the client to verify they understand the information correctly or a task the participant needs to perform.

that should all look very familiar to you. it is the nursing process adapted to a teaching plan. honestly, i got that information off a teacher's site which goes to show you that the nursing process isn't unique to our profession. we borrowed it from other professionals.
