NCP for Post Op Patient Fistulectomy

Guys can anybody help me in makin an NCP for my Patient who was already discharged( May Go Home) who underwent successful Fistulectomy operation. My patient was sent home the next day, and was given Meds to take for 7 days and was asked to go back for Check Up.

i can tell you are stuck with care planning because a care plan is based in the nursing problems the patient has, not upon the medical diagnosis or medical procedure that they had.

the reason your nursing instructors gave you lectures and information about assessing patients is because that information is crucial to determining what a person's nursing problems are. you need to go back and review what that information is that you collected. you, specifically, are looking for what was abnormal. this abnormal information becomes the evidence that will support the nursing diagnoses that you will end up choosing. your interventions and goals are based upon that abnormal assessment data.

discharge teaching includes addressing these subjects:

  • their diet
  • allowed physical activity
  • medications they need to take
  • any treatments, dressings and tests they need to be doing after discharge
  • referrals to any outside agencies or support groups
  • follow up appointments with doctors have been made and patient understanding
  • teaching materials and/or contact with outpatient professionals for continued care and teaching have been provided to the patient
