You need to identify the key point that guides your choice for answers and compre it to the rationale for the correct answer. Ask yourself why their answer and not yours. To figure that out, you have to think it through: think about the disease process; think about the patient's condition; then think about the outcome of your action based on those two factors in terms of helping the patient or harming the patient (physically and/or emotionally).
The NCLEX expects you to chose answers that are the most helpful to your patient. Look back at your incorrect answer and determine what the flaw is. You have to be able to pick out the flaws in the answers in order to eliminate the incorrect answers; and when you're just not sure of the correct answer, begin by eliminating the answers that you know are incorrect because they are out of place in reference to what the question is asking. Then choose the best​ of the remaining options.

Hi! I have my nclex scheduled for 5 weeks from now (I'll be working on a temporary licence in between). I've been using UWorld and Kaplan. I can't manage to score higher than 50%-52% on QBanks. I review the rationales and read up on it, but I've been studying for a month now and my results still aren't improving. Any tips to help me out? This is really making me nervous
