Ok, I was told that the nature of the questions the answer will never be "get someone else to do it". And that is fine, the questions should not be about passing the buck. But if a question is asked (doesn't matter what the question is, but here are the choices for answers).
A. Perform a heart transplant
B. Lobotomize the Patient
C. Remove the kidney
D. Have the Physician do it
Before everyone jumps all over me on this; the reason I am asking is to establish a general rule for how to look at NCLEX questions. We all know that we don't do A,B, and C... (I put them in there to be ridiculous) however, I know the answer can't be D because NCLEX type questions "Have the Physician do it" will never be correct. So if I take D away as being a possible answer, I must have missed something when being taught the other 3.
So in a round about way my question is, if one of the options on a test is "Have the Physician do it", can I take for granted that there is no way that is the right answer; and that regardless of how wrong the other options may seem, one of them must be the correct one.

No. The answer is *usually* not "tell the physician," but if all your other options are illegal (beyond your scope of practice) or put the patient in more danger, then the answer is going to be "tell the physician," etc.
I just had an ATI question like that last week and the answer *was* "tell the physician." It's a long shot; there are no guarantees.

I hate to tell you, but there are always exceptions with NCLEX. That's why it can be so frustrating & confusing.
