Hi! I downloaded the application packet from IDFPR and would like to clarify how to fill-up the 4-page exam application specially for the non-US educated RNs. BTW I'm from Philippines and I'm just making sure I have everything correct or else the processing will be delayed big time.
On the 1st Page Part II, how did you fill up the "Title" or do we even have to fill it up since we don't have a license from US yet? And if we are going to fill it up are we just going to write "RN" or really spell out "Registered Nurse"? And on the permanent mailing address did you write the name of your province? because it seems I didn't have enough space for it. And where did you write your "Zip Code"? Do you think I write it correctly from the sample below?

On the 2nd Page Part III, how did you write the "Type of Degree Earned"? Did you only write "BSN" or really spell out "Bachelor of Science in Nursing"? And under the "Specialized Training" section did you write anything in there? If yes, did you write the hospitals where you went on duty when you were still in college or only the hospital where you volunteered after you got your local license in the Philippines like in sample below? In addition are proof and certification from the institutions needed?

And on the 3rd page Part IV, is it really required to fill up the "Record of Licensure Information" page even though we don't have a license from US? If YES, on which part? Will it be under the "State of Original License" or "State of Current Licensure where you most recently have been practicing"? Or do I have to fill it up on both sections?And ong Part V, do I have to fill it up as well? If yes, is the "Name Of Examination" that I write from the sample below correct for Philippines NLE?

Oh man. I suggest you type that stuff up on a PDF processor. Don't know if you downloaded those forums from the Illinois website or not. Are they current? It might just be the scanner, but they seem really old to me.
Anyways, hopefully an admin moves this to the Nurse Registration forum, you'd get better help there.

Yes TheAverageMan, I downloaded it last month (November 2014) from IDFPR. I scanned the pages and rescaled the pictures to a lower resolution so that anyone can view it faster and I tested it before posting it here so I believe its readable enough
