Sometimes at our place some of us "forget" to sign a narcotic off but go back later like the next day to sign for it, does changing the dates on a few to make it coincide appropriately make it a serious error, is changing dates that much of an issue? We also have computer MARS that indicate when they were given as well
Yes, changing dates on the narcotic count sheet is a big deal.
If it were me, I would treat it like a late entry in my documentation and date/time the entry for when it was written, with a comment that it is a late entry and include the date/time the medication was actually given.
