Guys i'm a nursing student from the Philippines.If i may ask help from you guys in making a nursing care plan.Our Clinical Instructor gave us this situation.
Our Clinical Instructor gave us a situation about a TIA patient.Im not that good yet in making nursing care plans.In fact this is my first time to make one.
Sir with your kind graces,can you share me some of your knowledge and help me make one.This is the Situation
42 year old male patient was rushed in the hospital due to loss of consciousness.Attending physician,Initial diagnosis of Transient Ischemic Attack(TIA).Vital Signs upon arrival
Blood Pressure: 250/190
Respiratory Rate: 12
Pulse Rate: 135
Noted for drooling of saliva,hemiparesis.Doctor ordered for the following LABS. Stat CBC to include APTT and PTT,diagnostic test for STAT Scan.Hook up to IVF at plain NSS 1L to run for 20 drops per minute.
Can any one of you guys help me in making a nursing plan for this patient.In fact our C.I instructed us to hypothetically make a nursing plan that will show signs and symptoms of Transient Schemic Attack and appropriate Nursing Interventions.Id love it if you can help me guys.
This is my first time making a nursing care plan and i really need all the Help.

this is the sequence of events:
[*]using that abnormal data, choose nursing diagnoses. use a nursing diagnosis reference.
[*]finish care plan by writing goals and the nursing interventions (nursing treatment) for each abnormal data item that supports each nursing diagnosis.
