Hello everyone..I'm so lost. My situation is I was a Illinois resident when I graduated nursing school. I took my Nclex in Michigan. I applied for a Illinois license becuz I didn't realize I was relocating until after I scheduled my test. So I have taken and passed boards on the the 5th of September. I was told they mail out my license # and I have to pay $50. Well its going to an old address. I have tried to contact people regarding this situation at the IBON but no answer and no one has returned my calls! I want them to mail it to Michigan so I can pay for my Illinois license and then apply for Michigan by endorsement. What should I do? Help please!!

After you take the test they mail you a piece of paper that tells you if you passed or not. If you pass the paper says to fill out the bottom of the form along and send it with $50. After they process it, they send you your license with license number. You may be able to get the form on the idfpr website. The license number can be looked up using idfpr license lookup for Illinois. Check with michigan on what they need for license by endorsement, you might not need to physically have the license to apply.
Thanks...but with Illinois they won't put your name or license # up until you have paid the $50.
Did you contact continental testing 1-800-359-1313, 708-354-9911.
I looked over the paper and their the one who mail out the forms.
Maybe they can mail you another one to your new address.
