mental health assessment

Hi,

I am a nursing student and I need help conducting a mental health assessment, if anyone could give me a brief sample of a therapeutic dialogue for mental health assessment between the nurse and a schizophrenic patient, I am not sure how to ask the questions re: Appearance, Behaviour, and Cognition.

Any help would be appreciated,

Does your mental health nursing textbook not give an outline and samples of questions/interaction? What about your instructor?
