Hey all,
I was wondering about the responsibility of scheduling of first and subsequent medication doses in EPIC.
Who is responsible to ensure that medications due time is entered correctly?

In our Hospital pharmacy enters the times, but I do verify that new orders are in there correctly

Does the pharmacist have the privilage to change the schedule of medication. For example, if the order is daily, can the pharmacist change it to daily before breakfast, or can she/he change it to nightly?
Yes, in our facility only pharmacy can make those changes, but I often ask them to make a time change for one reason or another based on individual patient needs. Certain meds need to be given at specific times so they wouldn't be changed.

In ours, the provider's order determines the default times. So if they order it Once Daily Standard, it will default to 0900. Before Meals defaults to 0730, 1130, and 1700. If they just order it Every 4 Hours Starting Now, the first time will be a 15-minute increment from Now and then every 4 hours thereafter. Our pharmacy will adjust times for antibiotics (if they are ordered q8 but the patient just got a dose in the OR, pharmacy will adjust the post-op orders). Nurses can use the Adjust Times feature in the MAR to move times, but I will only do this within a small range. If the patient says they take a med at night that is ordered in the morning I let the provider know and change it.
