I need other nurses help...

I am an LPN and on the MMAC my project is gathering info on medication errors and why they accured. I need from my fellow nurses is this: Please leave a message regarding any medication error you know of and why it happened. Ex: Restoril order Resperidal given anything will help with this. Other nurses are the best source I could ever have.

Thanks for your help.

Huq. I should have said not just the nurses fault. Usually for a mistake to happen there are many contributing factors. Usually one person is not to blame.

However, as the patient, I can attest that Tylenol is not the same as Tylox!

You can do a search on medication errors on Google.

Some good websites are:

http://www.ahrq.org

http://www.mederrors.com

http://www.npsf.org

Do a Google news search on medical errors.

 every nurse should read & check the name of the medicine (generic name), its indication, dosage & contraindication, etc. before administering to the precious patient.

if the handwriting of the doctors seems hard to understand, contact that doctor, or ask or even seek advice to ur co-nurse & read it again. DOUBLE CHECK or MORE... the medicine.

Remember, an ounce of knowing is better than a pound of just looking.:)
