Hello! I have been practicing questions for the nclex using NCSBN and Kaplan.
My question is on medication errors, on one of Kaplan's questions:
(SATA) Nurse administers medication. While documenting the administration, the nurse realizes an error in administration. What actions must the nurse take?
Correct answers: evaluate the effect of the med, notify the pcp, notify the patient of the error, complete an occurrence report.
As I recall (no longer have access to NCSBN), we do not notify the patient if there was a medication error, I also vaguely remember it being on an ATI question during school, to not let the patient know of the medication error.
Please correct me if I misunderstood the way medication errors are handled in the "nclex hospital".

Wouldn't it be unethical to not inform the patient?

One of the major ethical principle in Nursing is veracity which means that health care professionals have to be honest and trustworthy when dealing with patients. Failure to deliver bad news is not an acceptable reason to hide this kind of information or to be untruthful.
However, in the real world, it is a different ball game. follow your hospital policy!!!

You should be able to notify the Pt just don't ever mention an occurrence report in the Pt's chart...
