2017 Medicare/Medicaid RN & LPN Providers (Independent Contractors aka Private Duty Nurse)

I am a LPN with an NPI/Medicaid #. I have worked as an Independent Contractor aka Medicaid Private Duty Nurse. I'm thinking about possibly traveling (nursing) for a higher rate since some cities have a shortage of nurses with NPI/Medicaid #s. I've learned, not all Medicaid Nursing Pay Rates are the same with different counties or cities. Please share your nursing title (RN or LPN) & your location Medicaid Pay Rate.

I am a medicaid provider RN on Suffolk county, LI NY. I get pain a little over $42/hr (including fragile child 30%) with one patient and just over $40/hr with another. I have also seen slightly different amounts for different kids within same county.
