Medicare Daily Charting

Legally, do you have to do Medicare A, B, C, D charting Q shift or daily for Tennessee?

Since Medicare is a FEDERALLY regulated program, the rules are the same state to state.

Med A requires (at least) daily documentation. Most places document every shift.

Med B requires a weekly note which should address the reason the person is being seen under their Med B benefit (slp 3x weekly for safe swallowing strategies) and how they are progressing.

Med C and D cover only pharmacy expenses and do not require any charting as such.

Thanks for your response. I've searched all over under "medicare charting" and have not been able to find anything clear on whether it was per shift or per day.

I I work long term care and medicare charting is done once a shift we use flow sheet with everything on it.
