pt falls

  1. I know this has probably been discussed ad nausem but I am new to this site. What does your facility have in place to prevent pt falls? What kind of screening tools q shift or on admission. We have been suuccessfull where I work but it is time to reevaluate and tweek what we have. What has worked for you? Thanks

    The last time I had an actual assessment for falls risk was working in psych. It was part of the electronic assessment instrument. I think it asked about weakness, type of meds where orthostatic hypotension can be a side effect, level of nutrition.

    What we put in place to prevent falls were using the yellow star on their door and on the board to remind us to take special care with them, educate them about asking for help, provide walker as necessary, etc.

    This probably doesn't help much, but it's a start.

    Have you searched the web yet? There is usually a load of this sort of info out there.....
