Medical clearance/brief history for cosmetic procedures

I am planning to work in a cosmetic clinic and the duty is just to take a brief history and sign off the medical clearance of the patient for procedures like botox, fillers, etc.

1. will I need a ?

2. Do I need a standardized procedure for the job?
