I work at 2 different cooporations. I was wondering if there is a general rule about filling out section B, C and J for the resident interview when the resident has an unplanned discharge and the interview is not made. At one of my jobs I just put 0 (rarely never understood) if the interview is not complete (company policy).

With the updated D/C assessment version in April, you have the option of checking if D/C was planned or unplanned in section A (sorry, don't remember the exact #). If unplanned, all interview sections are voided and are not required. There are many more changes in MDS since April, not just in D/C......

What about a 5 day assessment that is done prior to the admission. Sometimes I am unable to do the assessment until about 3-4 days later and they discharge.

Combine the 5-day and DC. Like Maga63 mentioned, if the DC is unplanned, you need not complete the resident interview portions.

When an interview is required yet not obtained, you can dash-fill the interview (0 = resident is rarely/never understood is not an appropriate answer). The "staff assessment," however, must be completed.
