But I am probaly making it too complicated.

I have seen titration protocols. I will use Heparin as an example....

A initial bolus of Heparin calls for (example): 13/kg = units to give.

I have always been taught that anytime there is a slash, it means to divide. Why does the / represent multiplication here?

And in my same med book a few pages over, it lists the standard calculation for how much of (whatever drug) to give by the formula:

Dose ordered/dose on hand x total mg/tab = how many tabs to give.

SO the / in this case does represent divide.......
