Maternal Care plan: NDx priority

I'm writing a postpartum careplan for a C-section...and am almost done. I need help with prioritization of the nursing diagnoses that I have applied to the plan of care. Any help/feedback would be appreciated.

I have the following NDx:

1. Acute pain r/t surgical incision (Cesarean section) and fundal contraction (postpartum) @font-face { font-family: "Cambria"; }p.MsoNormal, li.MsoNormal, div.MsoNormal { margin: 0in 0in 0.0001pt; font-size: 12pt; font-family: "Times New Roman"; }div.Section1 { page: Section1; }

2. Fatigue r/t pregnancy and recent Cesarean section @font-face { font-family: "Cambria"; }p.MsoNormal, li.MsoNormal, div.MsoNormal { margin: 0in 0in 0.0001pt; font-size: 12pt; font-family: "Times New Roman"; }div.Section1 { page: Section1; }

3. Knowledge deficit r/t fundal massage and potential complications @font-face { font-family: "Cambria"; }p.MsoNormal, li.MsoNormal, div.MsoNormal { margin: 0in 0in 0.0001pt; font-size: 12pt; font-family: "Times New Roman"; }div.Section1 { page: Section1; }

4. Risk for infection r/t inadequate primary defenses; broken skin @font-face { font-family: "Cambria"; }p.MsoNormal, li.MsoNormal, div.MsoNormal { margin: 0in 0in 0.0001pt; font-size: 12pt; font-family: "Times New Roman"; }div.Section1 { page: Section1; }

5. Impaired mobility r/t weakness & fatigue, spinal anesthesia, etc......

I would list them as follows:

1. Risk for infection

2. Acute Pain

3. fatigue

4. impaired mobility

5.knowledge deficent

the infection risk is the top priority because it could lead to a secondary problem. acute pain is next followed by fatigue (pain can make you very fatigued) then impaired mobiltiy ( she is in pain and does not want to move because it hurts) then finally knowledge deficent would be last

Over all the nursing Dx's you chose where right on target! Good luck and hope I helped somewhat
