Maryland Board of Nursing graduation confirmation form. What do I send???

Hi everyone! I recently graduated from an LPN program in NY but since I'm moving to MD, I applied to get licensed in MD. I was sent the application for my NCLEX PN and I mailed that in, along with copy of the cards with my fingerprints, as well as a check for the payment. After I graduated in June my school is suppose to send confirmation that I have graduated however the receptionist at my school has not sent anything in because she says that she thinks there is suppose to be an actual confirmation form that my school fills out since that's what New York has. I told her that i didn't see that form with my application and I have been trying to reach someone at the board of nursing for weeks. My school has not sent anything at all because they say they don't know what the Maryland Board of Nursing wants for confirmation. I was just wondering if anyone knew if there really is another form that my school needs to send in or is it just as simple as them sending in my transcipts and a letter from my director saying that I have graduated? I would appreciate any information since this is holding me up from being able to take my test and get licensed in MD. I have sent emails and left messages on diffferent voicemails and still have not received an answer. Just feeling a little discouraged right now.
