LVN having problems getting the charting rules for Medicare billing

18 years working as an LVN. For the last six months HHC. I am having a problem getting the charting rules for Medicare billing. Never in my 18 years have I ever been so frustrated. It is either not enough info or too much info. There's no in between, and the Co. keeps changing what they what in charting. Most of the LVN I work with are feeling the same way. Can or does anyone have advice or direction.

    I am new to home health and struggling with this too. It's so different from charting I've done in other areas. I haven't run into the too much information yet. Does your agency give you a template to use?
