They took forever on mine so I emailed them and I got it back 2 days later.

is anyone else having trouble with submitting the application and the nurse evaluation to the link (upload documents here) on the nursing website? it keeps telling me "this form is currently not accepting responses" am I doing something wrong?

are they pdfs?

Hi all, 

Did you enter all numbers of your social security in the application? Not sure if our application will be rejected if we only put the last 4 digits. It's sensitive information, so I'm not sure how to complete this section.

The same was happening to me so I switched to incognito browser and was able to edit the form!

Does anyone know who to email if you haven't received your prerequisite evaluation yet? I wasn't able to find any info about this on the website. ?

Ooo I think you are a little too late :(. Applications were due yesterday and the evaluation had to be done before the 25 of September

Oh okay! So you go on the website, and it will list all the steps you need to do prior to filling out the app. Under that first step, it will tell you to fill out the evaluation form that you need to download and fill it out with all the information  they will ask you such as your grades, student ID, etc. They will ask you to also submit your transcripts as well. The way you submit the evaluation form is by emailing it? I don’t quite remember exactly. And yes, the evaluation is mandatory as it goes along your nursing application. If you are a non LACCD student, you will have to submit official transcript to admissions and records as well.

Yes. LAVC is purely on a lottery based system
