Looking to revise a policy-need help!

I am trying to revise our policy reguarding how often we do vitals and full assessments on mom's and babies. I would like to know what everyone else is doing. So, how often do you do vitals and full assessments on moms (vag vs C-section vs PPTL) and well babies?

in that case, I peek in the room, and if mom and babe appear asleep and doing well, at 4, i go away and chart:

appear sleeping, eupneic and no apparant distress

then i do vitals about 5:30 to cover the requirement to do 3 sets on my 12 hour shift.

i do try not to disturb people too much in meeting our standard of care. hope this helps.
