I would like to ask if AZ included in the concurrency issues like Ca. I would greatly appreciated for your information.

These are states having concerns over concurrent theory and practicum. The last I checked.

Yes, they do, but there's been a recent posting that Joshbert here just received his ATT in AZ. Again, AZ is known state with the concurrency issue but not actively enforcing. Unless you feel your courses are concurrent, I know for one of my batchmates, she was denied in AZ (and CA). So it really depends on your transcripts.

You present an interesting case. I think while the CA BRN is looking at those that graduated with their BSN from PH post 2004, there was a recent poster who successfully endorsed over her RN license into CA without any problems and she took the NCLEX-RN out of CA as well.
I can't without any assurances say that the CA BRN may or may not grant you the ATT for the RN exam given that you're borderline with the 2004 graduation year. The only way to know for sure is to simply submit all the documentation that's asked of you.
I'm not sure about the scrubs requirements if they weren't signed, as if one piece of documentation is incomplete, they could just reject the application due to others who have submitted false documents.
I'm not saying that you are, but the CA BRN is reviewing all PH applicants under the microscope due to the recent CA BRN bulletins of PH grads and nurses "buying" their way in.
Maybe you can write a letter of explanation to the CA BRN with your application. You should also have a copy of your PH nursing license and your valid S/S# to submit as well.
