Kaplan Admissions Test - Where to take if out of state?

I need to take the Kaplan Nursing Admissions Test in order to interview at an out of state school I applied to. However, I have no idea where I can take it and no idea how to sign up for it. Does anyone know where the test is given in general or in NY?
