Report is part of the ongoing assessment that the RN takes of his or her patient, and part if the initial assessment of the receiving RN.
Report is the synthysis of the patients condition and the care the patient has received.
The LVN does not perform "ongoing assessment" as authoriced in the nursing practice act.
That said we know experienced LVNs who are able to receive and give report. But they do not have the legal authority to do so. I think it best that the nursing team receive report together while LVNs and nursing assistants from the offgoing shify provide patient care.
In those cases when an LVN observed something it would be ideal for that LVN to report specific observations to the oncoming shift.

Hi,
I am a socal RN and have a question. Can LVN's in California take report (independently)? I mean legally, scope of practice, not implying they are incapable of it. This has been an issue with us because we sometimes have to give report twice because the nurse is an LVN and cannot check PCA orders with us etc.
Anyone know? THanks
