IV weight based heparin

When working with IV heparin-weight based, do you routinely weigh the patient and weight adjust the infusion or do you keep the original weight?

keep original weight. if you keep changing the weight you will be changing the dosing which will mess up the PTTs.

Specializes in Surgery, Tele, OB, Peds,ED-True Float RN.

We keep the original weight UNLESS it used for an extended period of time which is unusual considering the high risk of the medication. But there have been circumstances that it is required for a longer time (months) and after a few or we have noticed a big weight loss/gain, then we would adjust!

Specializes in ER/ICU/STICU.

Keep the original because when the next ptt results come in, you are going to be adjusting the dose based on the ptt.
